Membership Subscription Payment Form
Members may access and complete the ONLINE application form for Membership Subscription Payment form available on the MAICSA website. You will still need to print out and submit the hardcopies of the form, together with payment, to the Secretariat.
Alternatively, you may download a copy of the offline application form for completion and submission together with payment to the Secretariat. Photocopies of the form are acceptable.
